Resumen
Vamos a realizar aquí una revisión de la historia de la cirugía del músculo oblicuo inferior, con especial énfasis puesto en el procedimiento de transposición anterior (TA). Discutiremos también en profundidad el origen accesorio de las fibras temporales e inferiores del oblicuo inferior a consecuencia del procedimiento de transposición anterior, y mostraremos la naturaleza anatómica y fisiológica del fascículo neuro-fibrovascular (NFVB) y las fundamentales relaciones de la unión mioneural. La tranposición anterior y nasal del oblicuo inferior invierte los vectores de fuerza de sus fibras postero-temporales, pasando de ser un elevador y exciclorrotador a convertirse en un inciclorrotador, depresor tónico y antielevador. Presentaremos una variedad de situaciones clínicas que podrían ser tributarias de este procedimiento.
Inferior Oblique Muscle Surgery
Summary
We will review the history of inferior oblique surgery with particular emphasis on the anterior transposition procedure. We will also discuss in some depth the ancillary origin of the posterior temporal fibers of the IO muscle following the anterior transposition procedure. The anatomic and physiologic nature of the NFVB and the landmark relationships of the myoneuro junction will be demonstrated. The anterior and nasal transposition of the IO will reverse the vector forces of the posterior temporal fibers from being an elevator and extorter to being an intorter, a tonic depressor and antielevator. Several clinical conditions that may warrant this procedure will be presented.
